Austin Mike first alerted me to this issue. I have one as does he. He may be able to shed more light regarding the origin than I am. My understanding is that it is a career contemporary piece--likely the 60s given its Mantle and Maris. I believe it was inserted in a package--hence the rounded upper corners--but don't know what package. It's not a particularly rare piece but it is cool. I think they sell around $30-40, so, with some time and searching, you should be able to get a reasonably priced one in good condition.
