Default sending this one to PSA - how should I fill out the form?

Posted By: Joe D.

I just joined PSA (2-year) and will be sending in 12 cards for submission (11 golf cards and this one football card)

I am not too familiar with the card / issue - as this was given to me years ago (I did not buy it)

Issue? value for PSA form?

edit to add question mark in title.

the value is just for shipping insurance purposes. Put down whatever is reasonable.

Thanks Anthony and John.

Knowing that its a $10 card... I probably will choose a different golf card to slab instead.
