Default Another puzzlement...

I purchased this card some time ago and promptly lost all info on what it is. Anyone have an ID?

On the back of the card where it says "Groundball to Sileski... back to Sileski for the double play", there is no record of a Sileski that ever played in the minors or the majors according to baseballreference.com, for what it's worth. Edited to add, I'm guessing it's from a collectors issue made in the late 70's-early 80's.
