Default Card ID Help Please!

I am having trouble identifying this card. Any help would be appreciated. It measures 3.5 x 5.25 inches.

1975 Sports Hobbyist Team Composite (hence the "SH" in the corner by the #)

The Sport Hobbyist was an early hobby periodical.
