Well, I figured out how the Sunnyvale Farms plaque ended up on my Seaver checklist. It's listed in the Sports Collectors Bible. It was given an ACC designation of F-505. The SCB claims that the set includes only Mets and Yankees players. They included a checklist, but they list the dimensions of the cards / plaques as unknown.
