Default Help with PSA Autograph Submission

I apologize in advance if this thread is duh-worthily obvious, but I have an easy question or two regarding some vintage autographed cards I want to have slabbed/authenticated (not graded). In filling out the PSA autograph submission form (don't know why, but I blocked out the player names for some reason), I've run into a gray area.

1. If I have four (4) cards of the same player, but from two or three different years/sets, do I group them all together like the '4' put into slot #1? Or do I list each card separately, or by year/set?

2. If the answer to question #1 is list the four cards together in the same slot, how do I determine 'Declared Value'? Do I multiply my 'value' by four (thus creating an overall value), or do I enter the value for one card there alone and PSA multiplies it by the quantity? It seems that it most definitely should be the former, but I want to make sure it is done correctly, as I'd hate to enter the large overall value number and then have PSA times that amount by four and hit me up for more money. Yowza!!
