Sometime next month I plan to release a second edition with the errors mentioned on this thread (as well as a few other minor errors that I have found) corrected. One reader has tentatively offered to host the second edition .pdf on his website for free download--which would be great from my perspective. If you can wait a few weeks maybe this will be a better solution for you than attempted modification of the first edition .pdf.
