I believe from reading my 1868 "The Game of Baseball" by Henry Chadwick that the catchers in the 19th century did NOT stand right behind the batter.

The diagram and drawings show the catcher standing maybe 20 feet behind home plate ... and the umpire even further back.

This would explain how they survived!!
