FYI there are only 4 D311's (1 PSA & 3 SGC), which meet your requirement.

We have one of these cards in our collection. It is a 1911 D311 Pacific Coast Biscuit,
Pfyl SGC 80, Ex/Mt6. We won this card in a Goodwin auction in mid 2006.
