Hi John. Yes, this is a dixie lid premium. My understanding is that you needed to send it 12 dixie cup lids to get a premium. Each premium came with holes punched in it so you could put it in an album.
Dixie lids with movie stars, athletes, etc. were so popular that this promotion ran for something like 25 years. The only two football lids are from the 1938 set and include Nagurski and Baugh. There is a premium of each player. I have no clue why they colorized his uniform as they did.
