People do care about the large/small letters. The prices paid do reflect a difference. It isn't just the size of the letters - the style of the images are quite different between the small and large letters. The images on the large are darker and more realistic. As a generalization, the large are scarcer and more valuable, but that varies from subject to subject. Here are my Speaker P2s: large is ungraded, small is graded - you can see they are quite different:
