Far as I know, they originated in Thailand. Most of the ones I've seen came out of Thailand directly (I got my first one from there) or via a UK seller. I had a big lot from the UK go missing in the infamous Jamaica, Queens, NY, mail center some years ago.
I corresponded with Martin Murray about this set several years ago and this was what he thought: "I do not think that this is of British origin, since otherwise many more would turn up over here. It is likely, as you surmise, that they were issued in a Far Eastern country and prepared there, for goods destined to be sold to Western visitors." The several I have are on your list.
