There's a column for "player name" so I would think the title would go there. Of course, it doesn't fit so maybe you could put it under the "comments" column. In the end they will figure it out. Just make sure you give them the proper designation. |

Personally I do not like the online forms, so I make an Excel Sheet and attach it to a submission form when submiting. |
