Default Help with IDing this Schmidt issue

Hi Guys:

A friend sent me the below asking for help. IDing it. I can not help, but hoping someone here knows.

I am waiting for more info as to size, what's on the back etc. Will fill that in when received, but hoping someone knows just seeing it.

Looks like a MSA issue but I've never seen that one before.

I commented on the other thread too, suggesting it appears to be a cut out from one of those MSA-like placemats or checklists for slurpee cups (or similar). I have no backing though, just a guess.
