It's still a unique card, but PSA has the year wrong, it's probably from 1972 or the next couple of years. He didn't change his name until the 1972 season, for one thing. For another, I've seen pictures of an original 1969 basketball box from which these cards were but (own a couple of cards from it), and there is no Kareem. This card was issued later.

How were these issued?

These are on the sides of shoeboxes. The set is commonly listed as a 1969 set but a look at the checklist disproves that date. My guess is that they were issued for several years through the late 1960s-early 1970s. The Kareem proves it.

Most of the cards were issued on the first box (a basketball box not a shoebox), which I think is indeed 1969. I have to see if I still have my pictures. A couple like Kareem were issued later.
