A late 1980s early 1990s issue of Baseball Cards magazine. They often had similar cards as a cutout insert in the magazine.

As correctly pointed out, this is not one of my fantasy/custom cards . . . but close.
I was publisher of Baseball Cards magazine in 1986 when this was part of a three-card insert. Read the blog post linked above for details as to why the Mantle doesn't have a 1988 copyright date as intended. Also sorry to report that I no longer have panels available as offered in the blog post.
