PSA will reject as altered or label a card altered for as little as an erased but visible pencil mark...but will qualify the card with an unerased pencil mark as marked.
 Actually happened to me with a Ted Williams card I sent in that had an erasure mark on the front where the penciling was clearly visible anyway.

Might also be a filled in pinhole at the top center.

That card gets my vote as one of the most aesthetically pleasing cards in all of the hobby, if not the most.
