I distinctly remember sending away some wrappers and I believe .25 cents or some amount like that and receiving an uncut sheet like this. I remember having to cut my own and no matter what I did I could not cut them straight.

I bought a collection years ago that included the blue checklists. Each had actually been used as a checklist by the owner, and you could see they had been crudely ripped from a full sheet. I kept them and haven’t come across any more in better shape.
