There are two documents included: 1. The actual stock certificate (approximately 4 x 6) with printing on both sides. They are the top picture on the left and the large picture on the right. 2. The check register receipt (approximately 4 x 3) which is blank on the back. That is the bottom picture on the left. Best, Steve
