It's either June 6 against the Reds off of Tony Cloninger or June 29 against the Cardinals off of Mudcat Grant.

Ernie also could have hit that one off of Dick Kelley on May 13, 1969 in a 19-0 romp over the expansion Padres. The long sleeves and the crowd apparel might point to a game from early in the season.

You’re right, I missed that one and you are correct it’s almost certainly that one because of the date. All three happened in the bottom of the first inning.
