Star Player Candy?

Ok guys and girls...I used the search function and found some info but not what I was looking for. I have 2 questions about this issue.

1. Does anyone know who, or where, these were issued specifically? All I could find is "possibly the midwest" since a few groups were found there.
2. Does anyone know where the name "Star Player Candy" came from?

I have been researching the origins but am wondering if anyone knows more?
